1. Purpose of Media Collection

During services, fellowship gatherings, community food relief banquets, youth initiatives, outreach missions, and special events, Cherubim & Seraphim Movement Church (King of Glory Model Parish) Australia Inc routinely records video footage, captures photographs, creates audio recordings, and broadcasts live streams.

The church gathers these media assets to highlight congregation life, honor community achievements, broadcast worship services live, and advocate for its charitable programs and ministry mission across public and digital platforms.

2. Scope of Permitted Use

By executing this release form, you authorize the church to utilize photographs, audio files, video clips, and livestream recordings featuring yourself (as well as any dependent children listed below) for the designated ministry channels:

• The official church website (www.kingofglorycsmcaustralia.org)

• Official social media outlets representing the church (e.g., Facebook, Instagram, YouTube)

• Continuous global and scheduled worship service livestreams

• Print media including community outreach leaflets, banners, brochures, and publications

• Ministry archives, annual summary reports, and grant or community partner documentation

3. Terms of Consent & Release

1. Royalty-Free Permission: I confirm that all media will be published without expectation of royalties, financial compensation, or payment.

2. Ownership & Copyright: King of Glory Model Parish CSMC Australia Inc retains full ownership and copyright of all photographs, digital materials, video footage, and audio recordings.

3. Respect & Dignity: The church pledges to present all media respectfully and positively, upholding Christian values and reflecting its mission and community ethos.

4. Revocation: I acknowledge the right to modify or revoke this consent at any point by emailing church leadership at csmcaustralia@gmail.com. (Any revocation applies strictly to future publications and does not affect previously printed assets or archived broadcasts).

By providing my signature below, I confirm that I have carefully reviewed and agreed to the terms in this Media, Photography & Video Recording Consent and Release Form. I affirm that I am at least 18 years old or the legally authorized parent/guardian of the minor(s) identified herein.
